Medical Forms Now Available

Official BSA medical forms are required for many Cub Scouting activities. You can now download those forms directly from Pack 151's web site. You will need Adobe's free Acrobat Reader program in order to view or print the forms. You can download that for free at the Adobe web site.
If you have questions about which form is needed for your particular activity, please contact the POC for the specific outing.
You can always find more information and Scouting forms on the official BSA web site - www.scouting.org.
